Service requests
The application must contain the following documents:
* an application in which the name, surname, place obtained, full name of the relevant championship or competition, age group, personal identity number, declared address of residence, bank account number, telephone number or e-mail address, athlete's trainer's name, surname, personal identity number, declared address of residence, bank account number, bank account number or e-mail address, telephone number or e-mail address, given name, surname and telephone number of the applicant;
* the name of the sports game team, the location obtained, the full name of the championship or competition concerned, the age group, the name and bank account number of the sports club representing the team, the name, surname, personal identity number, declared address of residence, bank account number, telephone number or e-mail address of the coach of the sports game team, name and telephone number;
* confirmation by the higher body of the sport concerned of the place obtained or a copy of the diploma.Receipt of services
The cash prize shall be transferred to the bank account of the athlete concerned indicated in the application, withholding all taxes provided for in the legislation in advance.
